A complete record of your care and treatment is kept in our medical records department. These records are strictly confidential, and only authorized persons have access to them. Copies of your health records will be available for a reasonable fee and following completion of an authorization form. To request copies of your medical records, please download the authorization form below and mail it to the following address.
